Disaster Economic Injury for Business Request
The County of San Luis Obispo Office of Emergency Services (County OES) is working to gather information from businesses on the economic impacts of the Mud Creek Slide.
The Estimated Disaster Economic Injury Worksheet for Businesses form is used by the Federal Small Business Administration (SBA) to determine whether businesses impacted by economic losses caused by a disaster will be eligible for an Economic Injury Disaster Loan (EIDL) program assistance.
To be considered for EIDL, affected businesses must submit this form to County OES, preferably no later than August 18, 2017. County OES will forward the information to California Governor’s Office of Emergency Services, who in turn, will submit it to the SBA.
Note: This worksheet is not an application for assistance. If the County is found eligible for the EIDL program, County OES will provide EIDL application information when it is received from Cal OES and the SBA.
The form can be sent via USPS to:
County of San Luis Obispo OES, 1055 Monterey Street, Room D-430, San Luis Obispo, CA 93408
Or fax to: (805) 781-5005
If you have any questions, you can contact County OES at: 805-781-5011
